Judge rejects bid for dismissal of hospitals' Medicaid lawsuit ...

CONCORD ? A lawsuit filed last year by Exeter Hospital and nine other of the state's largest hospitals about changes in Medicaid policies and reimbursements will be allowed to continue.

On Thursday, U.S. District Court Judge Steven McAuliffe ruled against the state's motion to dismiss the suit. In his ruling, McAuliffe said he had multiple questions that still needed to be answered about the case and that a hearing would be scheduled on Nov. 1.

The 10 hospitals sued the N.H. Department of Health and Human Services, which administers the state's Medicaid program, over changes in Medicaid policies and reimbursements. One of the suit's core arguments is the state failed to provide hospitals with notice and an opportunity to comment on the reduced rates before they were finalized. The hospitals argue the state reduced reimbursements to accommodate state-budgetary preferences.

McAuliffe ruled the hospitals have proven they suffered hardships because of the cuts and are entitled to a hearing.

"(The) plaintiffs have made a substantial showing that hardship is being suffered by both providers and Medicaid-eligible patients due to the reduced rates, and that continuing enforcement of those rates, if unlawful, will at some point result in irreparable injury (e.g., loss of medical care facilities, providers and the concomitant inability of Medicaid patients to obtain needed care)," McAuliffe wrote in his hearing.

McAuliffe said he wants the U.S. Secretary of Health and Human Services to answer the following series of questions at the Nov. 1 hearing:

* Should the court stay the Medicaid reductions pending a federal review of the cuts.

* Do federal officials believe the proper Medicaid rules were followed by the state in making the cuts.

* Do federal officials plan to take action against the state over the cuts.

* Do federal officials plan on approving the cuts.

The biggest hit to the hospitals during the most recent budget cycle were changes made to the Medicaid Enhancement Tax.

The MET is a 5.5 percent tax on net patient service revenue on all hospitals in the state. Up until recently, the tax revenue was returned to the hospitals through the Disproportionate Share program, but those reimbursements were eliminated in the recent budget, resulting in $250 million in cuts to the state's 13 largest hospitals, including Exeter.

"While state budgetary concerns cannot conclusively dictate Medicaid reimbursement rates, they do play a significant and legitimate role in the rate-setting process," McAuliffe wrote in his ruling. "But, even where significant state budget issues arise, still, Medicaid reimbursement rates must be set by participating states in accordance with methodologies and standards that are published in a state plan and approved by the United States Secretary of Health and Human Services (currently through the Centers for Medicare and Medicaid Services), and those rates must meet minimum federal statutory standards."

As a result of the Medicaid cuts, hospitals across the state made numerous staff reductions. Exeter Hospital cut 110 full-time equivalent positions throughout Exeter Hospital, Core Physicians, Exeter Healthcare and Synergy Health & Fitness. It also closed Exeter Healthcare, a skilled-nursing facility that accepted patients who need long-term access to ventilators. Eight patients using the facility were transferred elsewhere in the state.

Exeter Hospital officials applauded the most recent ruling. "We're pleased the judge has recognized the legal merit of the case," said Exeter Hospital spokeswoman Debra Vasapolli.

Not so, says a recent study from the Georgetown University Center on Education and the Workforce and Civic Enterprises. The research found that despite the obvious erosion of many traditional routes to middle-class earnings for non-graduates (nope, I wouldn't bet on that factory re-opening in town), there are still 29 million jobs in America that pay middle-class wages (defined here as between $35,000 and $75,000 annually) that don't require a bachelor's degree. Forty percent of those pay more than $50,000 a year. (For New Yorkers and inveterate high achievers, $50,000 might not sound like a lot, but it beats the U.S. median.)

While in the past, blue-collar gigs were the route to the middle class for non-graduates, this study reveals that most of today's jobs for non-grads are now in service sectors like healthcare or white-collar office jobs.

So how do you find your way to one of these reasonably decent jobs if you missed the boat on college? The research suggests getting one of these gigs still takes some effort, with the center suggesting five paths to this sort of steady employment:

  • Associate's degrees. Half of associate's degrees are related to career-oriented fields, such as nursing, business, and information technology.

  • Postsecondary certificates have eclipsed associate's and master's degrees as the second most common post-secondary award after the bachelor's degree-about 1 million are awarded each year.

  • Registered apprenticeships. Nine out of 10 apprentices are men and more than half of apprenticeships are in construction.

  • Industry-based certifications such as Microsoft, Cisco, and CompTIA certifications are test-based postsecondary credentials awarded by employers.

  • Employer-based training represents the largest pathway at $454 billion of spending.

Of course, while it's great to get some counterweight to the general cultural gloom hanging over those who fail to make it out of a four-year college program, the authors of the study don't think enough is being done to help those who need to find a non-college route to a good job.

"Compared to other advanced economies, the United States under-invests in sub-baccalaureate, career and technical education," said Anthony P. Carnevale, the Center's Director and the report's lead author.

'Suspicious' fire guts much of Vallejo mayor's law office

Someone appears to have torched the law offices of Vallejo Mayor Osby Davis, fire and police officials said Saturday.

A fire that gutted the Davis' front office on Tuolumne Street across from the Solano County Superior Court early Saturday is considered suspicious, police and fire officials said.

"We're investigating it as arson at this time," Vallejo Police Dept. Lt. Sid De Jesus said. "We don't know for sure, but we suspect it's an arson case. Obviously, we're erring on the side of caution, working with (the) fire department.

"The cause and origin is yet to be determined, but because it's an office occupied by the mayor, we're looking at this from every investigative aspect that we can."

Reached by phone later, Davis said that to describe himself as "upset, is an understatement."

He said he was awakened at about 1:30 a.m. by a phone call from Fire Chief Paige Meyer, and knew it was going to be bad news.

"Any time the mayor gets a phone call in the wee hours of the morning, it's never good," Davis said. "I really don't know what to say right now."

Davis said he will carry on.

"I plan to keep going," he said. "This is a bump in the road, a temporary hurdle. And we'll get past it and continue to do what we need to do."

De Jesus said there is no indication "of a nexus" of any kind between the fire and the most recent officer-involved shooting that left one man dead and his family and supporters outraged. Arson is not a rare, though also not a common

crime in Vallejo, he said.

"We're not jumping to conclusions," De Jesus said. "Both (Davis and Micheal Thompson) are high profile people. It could be a disgruntled client of either attorney that occupied that office. I know the mayor is doing everything he can to stay on top of the latest officer-involved shooting, to address the concerns of the citizens, by soliciting an independent review which we welcome. It could be any number of things." Thompson shares the office with Davis, and has been a judicial candidate.

Last Tuesday, the Vallejo City Council unanimously agreed to seek an independent review by the state attorney general of the Sept. 2 fatal shooting by police of Mario Romero, 23, outside his North Vallejo home. Family members have strongly denied police accounts of the shooting, in which police claimed he reached for a gun in his waistband. It turned out to be a pellet gun, although family members denied that he owned one.

Calling the blaze, which appears to have started in the front waiting room area, "very suspicious," Fire Chief Paige Meyer said the single-story office building in the 400 block of Tuolumne Street, was about 50 percent involved when crews arrived shortly after 1:30 a.m. Saturday.

"We knocked it down within about 15 minutes, but the building looks like nearly a total loss," Meyer said. The contents, however, were mostly salvageable, he said.

Greg Marshall, the general manager of Premier Construction, the contractors hired by the city to board up burned buildings, said his firm got the call about 2:30 a.m. He said that since much of the damage to the building is from smoke and water, he's also been asked to salvage as much of the contents including legal documents, as possible.

Marshall said Davis was at the scene about 3:30 a.m.

This is the second incident in recent months in which the mayor has been the victim of a possible crime. On May 18, Davis' 11-year-old motorcycle was stolen from the City Hall parking lot.

De Jesus said police are asking anyone who may have been in the area at about 1:30 a.m. Saturday and think they may have seen something "suspicious in nature," to call the Vallejo Police Department's non-emergency number, 552-3285.

Biden, 69, seeks Florida seniors' votes for Obama

BOCA RATON, Fla. (AP) ? Joe Biden may just be the perfect pitchman for the senior vote.

Seeing Medicare as a window to gain new support, President Barack Obama's campaign dispatched the vice president to two sprawling Florida retirement communities Friday, hoping a white-haired 69-year-old running mate will be able to stir enthusiasm among seniors in Democrat-rich South Florida and tip the scales for the state's 29 electoral votes.

"It makes sense," says Charles Zeldon, a Nova Southeastern University expert on politics and voting. "He is one of them."

Well, at least closer than Obama, a generation younger.

That may give Biden an edge in helping the president chip into Republican Mitt Romney's lead among senior citizens, a key voting bloc not only in Florida but other battleground states such as Iowa and Ohio. The Democratic campaign doesn't expect to win the majority of seniors, but hopes that lowering Romney's totals could make the difference in close states.

Biden has spent much of the campaign trying to shore up support among white, working class voters, another group where he has a more natural connection than Obama. He's aiming to use his same affable, plain-spoken style to persuade older voters to back Obama.

He stopped by a local deli later Friday, where he greeted a man who informed him he had once shaken the hand of President Franklin D. Roosevelt.

"I'm shaking a heckuva hand," Biden announced. The man identified himself to reporters as Seymour Maiman, 85, of Ft. Lauderdale.

Biden hit all the expected notes before a crowd of about 850 at Century Village in Boca Raton, a popular campaign stop for Democrats. He spoke of Obama as a defender of popular entitlement programs like Medicare, and portrayed the plans of Mitt Romney and running mate Rep. Paul Ryan as harmful to seniors and their families.

"What they don't tell you, what they really don't want to talk about, is how they'd fundamentally change Medicare," Biden said. "They'd turn it into a voucher program."

When it comes to Medicare, the Republicans say the Democrats have it backward.

Ryan, speaking to an AARP conference last week, declared, "The first step to a stronger Medicare is to repeal Obamacare because it represents the worst of both worlds. It weakens Medicare for today's seniors and puts it at risk for the next generation."

"Who are you going to believe?" Biden asked on Friday after unleashing a barrage of criticism of Republican plans.

Ryan's Medicare proposals have been a frequent target for Democrats, but Biden went further, saying a vote for the GOP would also lead to hundreds of dollars in tax increases on seniors' Social Security benefits.

"They cannot possibly continue and add to the tax cuts for the super wealthy unless they eviscerate the rest of the budget," he said.

Again, the Romney camp said, Biden had it exactly wrong.

"Vice President Biden is using Social Security to fabricate the Obama campaign's latest false attacks," said spokesman Ryan Williams.

Inside the Century Village clubhouse, a Democratic-friendly but sometimes cantankerous crowd didn't muffle any complaints. "We can't hear you!" some shouted when the volume was too low. "Sit down!" when their view was obstructed. And, at least once, "This music is awful!"

But Biden was met with loud applause and beaming faces.

"I'm a Joe fan! I'm a Joe fan!" said Judy Cloutier, 66, who came to hear Biden speak. "He's such a down-to-earth man. He's like salt of the earth."

Dot Tillett, 67, said Biden was the right running mate for Obama, and a good choice to send to talk to seniors. "He's a perfect complement," she said. "He communicates well with people of all types. He's just that kind of guy."

Dorothy Schayes, 87, was less enthusiastic about the vice president, calling him "a joke." But deeply behind Obama, she hoped others were paying attention to the Medicare debate, which she said spurred concern for her 16-year-old granddaughter.

"I think there's trouble for her," she said. "So he needs another term."

At the deli later, the vice president also made a detailed sales pitch to a customer who had questions about Obama's health care overhaul.

"You will be able to get better health care than you get now in terms of cost for a lot less money," Biden told Steve Grossman, 39.

Younger people also are being enlisted by the Democrats in going after the elderly. The Obama campaign announced a new e-card effort asking young supporters to send messages to their parents and grandparents telling them their Social Security and other benefits are at risk under Republican plans.

Seniors, like people in most demographics, are largely decided on who they'll vote for in the election. So making sure supporters turn out at the polls is important for both campaigns. Nationally, the 65+ population votes at a higher rate than any other age group.

Florida has a higher proportion of people 65 and older than any other state, and its total population of nearly 3.3 million seniors is second only to California. An estimated 68 percent of seniors in Florida voted in the 2008 presidential election.

A national Associated Press-GfK poll earlier this month found 52 percent of seniors supported Romney compared to 41 percent for Obama. But Democrats have sought gains among seniors by criticizing Romney's plan for Medicare, and a Pew poll released last week showed older voters who rate Medicare as a very important issue supported Obama by a substantial margin.

Zeldon said the resurgence of the Medicare issue has given an opportunity to Obama with older voters.

"He sees them as a vulnerable population for the Republicans," Zeldon said. "When they raised the issue of Medicare, they gave the Democrats a gift, because it allows them to give their ideas about Medicare and it's an easier sell."

YouTube Blocks Contentious Brazilian political video

Straight to the point: Google?s YouTube has agreed to block a video clip in Brazil that has been the focus of legal action and, oddly, an arrest.

The company, in a statement, made it plain that when it receives a take down request that it finds onerous, it does legally protest. However, once that legal dance has run its course, if a court does demand that a clip be restricted, Google complies.

Here?s where things stand at the moment:

Late last night, we learned that our final legal appeal has been denied and so now we have no choice but?to block the video in Brazil. We are deeply disappointed that we have never had the full opportunity to?argue in court that these were legitimate free speech videos and should remain available in Brazil.

Free speech has been a hot issue recently, after a buffoonish clip lampooning the Prophet Muhammad led to global protests, riots and violence.

Google is taking a longer view on this issue, promising to ?continue to campaign for free expression globally.? In a time in which it?s all too simple to get caught up in the minutiae of the smartphone wars, it is important to realize how large a company such as Google truly is, and how complex their operations in fact are.

I?ll give the company the final thought:

Ironically, the user who published one of the videos has now removed it and closed their account ??showing just what a chilling effect these episodes can have on free speech.

Indeed. For shame, Brazil.

Dating & Relationships: Oh You've Got Games?

Games, we?ve been playing them since we were children and as adults we still play these games. ?Except as adults are games have become interestingly complex. ?Mommy?s pissed at daddy, she ignores him. ?Mommy spent too much money on shoe shopping, daddy yells and then spaces out into countless hours of football watching. ?Singles, well it becomes even trickier. ?You dance with the guy at the bar, just to make the guy you really want jealous? ?What?

A 5 year old would say, ?Well mommy & daddy just say you?re sorry like you tell me to do.? ?A 10 year old would tell the Single, ?Duh if you like the boy then just tell him.. Sheesh!? ?To children it?s probably common sense what we should do, but we as adults assure ourselves that these complex games are the proper antidote.

Someone recently asked me to write about ?Where games stop and true unconditional love commitment begins?.? ?Face it, to some degree we all play games, but I?m convinced we can all agree that the games are tiresome and often produce very fickle results. ?So why do we play these games? ?Why don?t we just talk it out, why not communicate how we feel?

We get hurt, we?re afraid of rejection, we sometimes feel so trapped we?re not sure how to communicate anymore, our anger gets the best of us so we don?t dare give our hearts a chance to speak and so the list goes on. ?I love being transparent, I love sharing how I feel, I often feel like it?s too much. ?In my dream world I would freely be able to communicate all that my heart feels. ?Reality is it?s not always the best idea, especially when someone doesn?t know how to handle it or when they?re not a safe person to do so with.

The truth is we have to remain vulnerable in this journey of love. ?When we find ourselves in situations where we feel compelled to constantly play games we must re-think where we are and if we want to stay in that position. ?Unconditional love takes its place where there is safety, where there is courage to break away from the lies and when we fight to remain true to our hearts.

Though we may all continue to play games for the sake of ?self preservation?, there?s a fine line when playing games becomes our way of existence in dating & relationships. ?For me I prefer truth. ?Sure tease me with the chase, I get it, but ultimately I want truth, I want substance, I want real love. ?Oh you?ve got games? ?Consider exchanging the game pieces for love, honesty and genuine emotions.

Netanyahu-Obama tensions nothing new: 5 low points in US-Israel ties

Will US-Israel relations fray over Iran? Not likely ? they've seen worse. Here are five noteworthy hiccups in the 'special relationship' since Israel's founding in 1948.

1. 1956 Suez fallout: Eisenhower threatens to withhold aid

When Egyptian President Gamal Abdel Nasser nationalized the Suez Canal in 1956, Israel invaded Egypt and quickly captured the vast Sinai peninsula. This provoked an international backlash. The United Nations Security Council issued a resolution calling on Israel to withdraw in exchange for a UN protection force in the Sinai.

Israel mostly complied, but President Dwight D. Eisenhower insisted that Israeli forces withdraw completely. In February 1957, he threatened to withhold more than $100 million in annual US aid if they didn?t.

Within less than a month, all Israeli troops had left, and by late March oil was flowing through the Suez once again.

While Eisenhower took a firm stance, believing that ?the future of the United Nations and peace in the Middle East may be at stake,? throughout the crisis he emphasized America?s friendship with Israel.

?I need not assure you of the deep interest which the United States has in your country, nor recall the various elements of our policy of support to Israel in so many ways,? he wrote in a letter to Israeli Prime Minister David Ben Gurion on Nov. 7, 1956. ?It is in this context that I urge you to comply with the resolutions of the United Nations General Assembly dealing with the current crisis and to make your decision known immediately. It would be a matter of the greatest regret to all my countrymen if Israeli policy on a matter of such grave concern to the world should in any way impair the friendly cooperation between our two countries.?

2. 1975 ?reassessment? of America?s ties with Israel

As elsewhere, many of the United States' moves in the Middle East in the 1970s were shaped by cold-war calculations. When Egypt?s relationship with the Soviets fell apart, Secretary of State Henry Kissinger saw an opportunity to pull Cairo more firmly into the American camp by helping it regain the Sinai, which Israel had recaptured in 1967 and successfully defended in the 1973 Yom Kippur War.

From 1973-75, Mr. Kissinger embarked on a flurry of shuttle diplomacy, securing a cease-fire and an Israeli agreement to pull back from the banks of the Suez. But in March 1975, while trying to negotiate a fuller withdrawal, Kissinger lost his temper with Israeli Prime Minister Yitzhak Rabin, who refused to cede strategic passes in the Sinai as well as oil fields that provided Israel with roughly 60 percent of its oil.

?You don?t understand, I?m trying to save you,? Kissinger reportedly said, according to an account in Commentary Magazine. ?You are making me, the Secretary of State of the United States of America, wander around the Middle East like a Levantine rug merchant?.Are you out of your mind??

The President Gerald Ford sent Rabin a letter informing him that ?I have given instructions for a reassessment of United States policy in the region, including our relations with Israel?? The US also put the brakes on an Israeli request for F-15 fighter planes and froze scheduled arms deliveries.

But US senators rallied in support of Israel, and while President Ford stood firm on withdrawing from the passes, he added enticements including $2 billion in financial aid to Israel. In September an agreement was reached, under which Israel and Egypt agreed that American ?technicians? and the UN would oversee the strategic passes. By the end of the decade, Israel and Egypt signed a formal peace treaty mediated by President Jimmy Carter.

3. 1981: Israeli strike on Osirak nuclear reactor in Iraq

Israel?s 1981 surprise strike on an Iraqi nuclear reactor known as Osirak put President Ronald Reagan in a tough spot.

While Reagan was a strong supporter of Israel, Iraq had recently become a de facto friend by going to war with America?s enemy, Iran. The fact that Israel had used US-supplied fighter planes to hit the Iraqi plant boded ill for that nascent friendship.

?Eager to dispel any semblance of collusion in an attack against America?s new de facto ally, Reagan delayed the delivery of additional jet fighters to Israel,? writes historian Michael Oren in his 2007 book ?Power, Faith, and Fantasy? on America?s involvement in the Middle East. ?He also permitted America?s pertinacious ambassador to the UN, Jeane Kirkpatrick, to confer with her Iraqi counterpart in drafting a Security Council condemnation of the raid.?

But the raid ?did not, in the end, impair US-Israel relations ? ? concludes Dr. Oren, who has since become the Israeli ambassador to the US.

4. 1991-92: Delay of US aid over Israeli settlements

Israeli Prime Minister Yitzhak Shamir, an ?migr? from Poland whose parents had died in the Holocaust, was intent on bringing as many new immigrants to Israel as possible. When the Ethiopian government collapsed in 1991, he had 14,000 Ethiopian Jews airlifted to Israel. And he convinced the US to stop accepting Soviet Jews as refugees, funneling more than 1 million Soviet Jews to Israel instead.

But all of this cost money. During the end of President George H.W. Bush?s term, Mr. Shamir sought loan guarantees of $10 billion from the US to help cover the resettlement costs, which included building new homes and roads, finding jobs, and providing services for the new immigrants. (The mechanism was akin to co-signing a mortgage, in which a wealthier party with better credit can help a party with more modest funds to secure lower interest rates.)

The Bush administration was concerned, however, that the funding would be used to build new settlements that could jeopardize peacemaking between Israelis and Palestinians.

In the end, Shamir ? a staunch believer that the West Bank and Gaza rightfully belonged to the Jews ? never accepted Bush?s terms to freeze settlement expansion in exchange for the loan guarantees. But in 1992, when Yitzhak Rabin replaced him, Bush agreed to the full $10 billion in loan guarantees in exchange for only limited curtailment of settlement building ? perhaps because he viewed Rabin?s centrist government as more supportive of America?s peacemaking efforts in the region.

The following year, Rabin signed the 1993 Oslo Accords with Palestinian leader Yasser Arafat.

5. 2010: Settlement flap during Biden visit

Despite the Oslo Accords, however, the population of Israeli settlements in the West Bank nearly tripled over the next two decades ? from 115,000 to more than 300,000 ? and the Jewish presence in East Jerusalem grew to more than 200,000.

Building in both areas, which Israel captured in the 1967 war, is considered illegal under international law and many see it as prejudicing an eventual Israeli-Palestinian peace agreement.

When President Obama took office in 2009, his administration called for a full settlement freeze. But in an incident widely seen as Israel thumbing its nose at that demand, Israeli officials announced approval for 1,600 new homes in East Jerusalem during a visit by Vice President Joe Biden in March 2010.

Secretary of State Hillary Clinton scolded Israeli Prime Minister Benjamin Netanyahu in a 43-minute phone call about the "insult" caused to Biden. While Congress gave Mr. Netanyahu a warm welcome when he arrived in Washington the next week, the White House departed from protocol and refused to allow photo ops or issue statements about his visit.

According to one account, Obama curtly presented Netanyahu with a list of demands and when he stalled, the American president abruptly left to have dinner with his family.

The next month, it was reported that Israel?s ambassador to the US, Michael Oren, told Israeli consul generals on a conference call that relations between the two countries were at a 35-year low. Ambassador Oren denied having made that remark, however, and both sides seemed to make an effort to smooth out the relationship.

While differing timetables over Iran have brought US-Israel tensions back to the fore in recent months, much of that has been attributed to personality differences and election campaigning rather than a fundamental breakdown in ties. Obama is on the homestretch of his reelection campaign, and Netanyahu is expected to announce early elections before the end of the year.

A New Wave of Digital Entrepreneurs (and Funding) in Latin ...

eMarketer projects that between 2012 and 2016, Latin America will be one of the world?s fastest-growing regions for online advertising spending, growing from $3.62 billion in 2012 to $7.68 billion in 2016, a 112% increase in just four years.

Yet, legacy monopolies like Televisa, creator of many of the world?s most popular telenovelas along with its competitor TV Azteca, still dominate media spending where 56% of advertising dollars go towards broadcast television while only 10% goes towards digital advertising.

Can today?s Spanish-speaking, social media-oriented start-ups break into the big time alongside monopolies to further drive digital growth in Latin America?

Social Media Taking Off
Another recent eMarketer report shows that five of the top ten countries that spend the most time on social media sites are located in Latin America: Argentina, Chile, Peru, Colombia, and Mexico. In addition, Mexico appears in third place among the top five social networking countries by consumer growth rate, behind India and Indonesia. Even with stratospheric growth in social and mobile media usage, those two categories represent only 6% and 5% respectively within all digital media spending.

Alejandro Fosk, comScore senior vice president of Latin America, says, ?Nearly 100 percent of the Latin American Internet population visits social networking destinations each month.? comScore?s latest research shows that more than 127 million Latin Americans ages 15 and older visited a social networking destination from a home or work computer in April 2012. And Facebook Accounts for 1 in Every 4 Minutes Spent Online in Latin America.

Infrastructure Challenge
Geoff Ramsey, the CEO of eMarketer, points out that a lack of infrastructure in Latin America and especially in Mexico hold back digital media growth. ?I hope that the infrastructure problems can be resolved so that digital media flourishes and Latin Americans have price points that attract the middle class, not just the affluent,? Ramsey said after presenting at the Internet Advertising Bureau Mexico?s annual conference. ?There?s a huge skew towards the affluent here.?

eMarketer shows that 41% of Mexicans have access to the Internet, well below other countries in Latin America such as Argentina where 54% of the populations has access to the Internet, for example. Ramsey points out that the difference in broadband is even more severe: only 26% of Mexicans have access to high-speed Internet versus 47% of Argentineans, a 21% difference.

When you look at mobile penetration in the region, only 55% of Mexicans have a mobile phone, again lower than Argentina with 79% penetration and even below the average for all of Latin America, which is 65%.

Investment Lag
If we look at Internet ad spending per user as a relative measure by country, you see that developed markets like the UK or the US invest $185 and $165 per person, respectively. Yet, in markets like Brazil or Argentina, companies only invest $24 and $20 per user. And Mexico invests even less: only $10 per user. Ramsey says, ?Even though you have fewer people online, Mexico?s online marketers are not investing as much per person as developed markets.? eMarketer finds a similar trend among other developing markets. China, for example, invests only $13 per Internet user.

Gian Fulgoni, the CEO of comScore who also presented at the IAB Mexico?s annual conference last month, says ?In 2011, Latin America online advertising grew at a rate of 42% with Mexico growing at 36%. There is much more room to grow since online advertising represents only 8% of the total media spend in the region.? comScore shows that Latin American online advertising spending came to $2.5 billion dollars, a 42% year-over-year growth rate.

Appetite for Start-Ups
With the explosive growth in social media in Latin America, clearly there is demand from a fast-growing digital audience to encourage Internet entrepreneurs in Mexico. Perhaps these start-ups can foster innovation, more competitive prices and even improve the digital infrastructure. Yet, the timing may be right for investors who are willing to look outside of Silicon Valley for the growth that Mexico and Latin America can provide. In addition, a healthy number of graduates in engineering from reputable universities like Tec de Monterrey provide start-ups with the talent they need, well below the cost of engineers in the U.S.

Techcrunch?recently reported that Alta Ventures closed a $70 Million fund to invest in Mexican technology startups. The article says, ?of all the countries benefitting from the investment, Mexico seems to be hottest, especially since its economy grew even faster than Brazil last year.?

And earlier this summer, 500 Startups? founder and former PayPal executive Dave McClure announced that 500 Startups would acquire Mexican startup accelerator Mexican.VC. (Read more about Mexican.VC in my previous article here.)

500 Startups??seed companies in Mexico include Wowzer, a video-screening tool for interviewing and recruiting candidates using technology, and Fontacto, which provides local phone numbers in the major cities across Mexico to small businesses without a local office.

Interestingly, McClure isn?t the only foreigner to invest in Mexico. Ulrich Noel recently ramped up operations in Mexico for Rocket, a privately held investment company based in Germany that rolls out Internet businesses across the world. He set up the company and hired 100 employees to launch an e-commerce website, Linio, in Mexico City this past May, according to a recent article in Expansion Magazine. Some people criticize Rocket for blatantly ?copying and improving? web business models while bringing them to new markets. The company has successfully copied eBay with Alando, Groupon with CityDeal and AirBnB with Wimdu, all in Germany. Now, the company hopes to roll out similar models in Mexico and use that as a launching pad for expanding into Latin America.

The fines announced Wednesday come as the league and its referees' union are meeting amid reports they are close to ending the lockout of the regular officials.

Belichick?grabbed an official's arm at the end of Sunday night's game after the Ravens kicked a winning field goal. Shanahan was cited by the league for "abuse of officials" in Washington's loss to Cincinnati.

"Whatever the league decides to do, I'll respect and just try and do my job,"?Belichick?said before his fine was announced.

On Monday, the league fined Broncos coach John Fox $30,000 and defensive coordinator Jack Del Rio $25,000 for berating officials during a loss to Atlanta on Sept. 17.

Hereditary Colorectal Cancer

"Mortality/Morbidity

Although not everyone who inherits the gene for hereditary nonpolyposis colorectal cancer (HNPCC) develops colorectal cancer, individuals with Lynch syndrome have a 70-80% lifetime risk of developing colon cancer. Of these cancers, two thirds occur in the proximal colon (proximal to the splenic flexure). In approximately 45% of affected individuals, multiple synchronous and metachronous colorectal may occur within 10 years of resection.
Other cancers associated with hereditary nonpolyposis colorectal cancer (HNPCC) (Lynch Syndrome) include the following:
  • Endometrial cancer: The lifetime risk is 30-40% by age 70 years. The average age at diagnosis is 46 years. Half of patients with both colon and endometrial cancer present with endometrial cancer first.
  • Ovarian cancer: The lifetime risk is 9-12% by age 70 years. The average age at diagnosis is 42.5 years. Approximately 30% of these tumors present before age 40 years.
  • Gastric cancer: The lifetime risk is around 13% (higher in Asians). The mean age at diagnosis of gastric cancer is 56 years; intestinal-type adenocarcinoma is the most commonly reported pathology, especially in Asian countries such as Japan, Korea, and China.
  • Transitional cell carcinoma: The lifetime risk is 4-10%. This principally affects the upper urinary tract (ureters and renal pelvis).

  • Adenocarcinoma of the small bowel cancers: The lifetime risk is 1-3%. These occur most commonly in the duodenum and jejunum.

  • Glioblastoma: The lifetime risk is 1-4%. Also known as Turcot syndrome, this is a variant of hereditary nonpolyposis colorectal cancer (HNPCC) (see below).

  • Malignancies of the larynx, breast, prostate, liver, biliary tree, pancreas, and the hematopoietic system are more common in patients with hereditary nonpolyposis colorectal cancer (HNPCC).

Fort Campbell Suicide Prevention Efforts Target a Killer ? Clarksville ...

While suicide has long been believed to be a silent killer, the command at Fort Campbell and the 101st Airborne Division believe the warning signs often sound the loudest alarms. Knowing those warning signs can mean the difference between life and death.

?We would like our soldiers to look at brain injuries and illnesses much in the same way they look at physical injuries and illnesses,? said Maj. Gen. James C. McConville, commanding general of the 101st Airborne Division (Air Assault) and Fort Campbell. ?Prevention and early intervention are the secrets to success. Our Soldiers don?t have problems going to see a doctor when they are physically injured or ill, yet they are sometimes reluctant to do so with behavioral health issues. Knowing this, we are putting embedded behavioral health teams at the brigade level to provide this service at the point of need and make it more convenient to get.?

As part of the U.S. Army?s Suicide Prevention Month, Fort Campbell and the 101st Airborne Division will host several events throughout the week to educate, train and prepare soldiers, families and civilians on spotting the warning signs of suicide. Community members will also receive briefings on how to live healthier by making better decisions on how to handle the stressors that come with Army life. The week culminates with a Suicide Stand Up Day, September 27th-28th, with training seminars led within soldiers? units. A wellness walk, September 29th, beginning at 9:00am at the Sportsman?s Lodge, will provide a healthy, family-friendly way to deal with any lingering frustrations or stress in the tradition of a German Volksmarch.

During the Suicide Stand Up Day, soldiers will participate in ASIST, or Applied Suicide Intervention Skills Training, a program aimed at educating community members on how to spot warning signs of risky behavior and how to effectively intervene, as well as group discussions with their unit leaders, and training facilitated by master resilience trainers. Units will also lead esprit de corps events for additional team building and to balance out the weight of the issues discussed.

Soldiers are not the only ones in training on how to handle stress and prevent suicide. In addition to suicide prevention training traditionally offered, families and Department of the Army civilians also received the ASIST training and are encouraged to participate in the Wellness Walk September 29th. Family members have also started to receive resilience training that follows the model of Comprehensive Soldier Fitness, balancing the mind, body, spirit, and emotions. Some units have even taken to combining soldiers and their spouses for couples? resilience training classes.

?Resilience training helps to empower the soldiers and family members at Fort Campbell. Resilience is not just about coping with the ups and downs of a deployment, but helps to teach skills that are useful in our daily lives,? said Jennifer Sztalkoper, one of the spouse master resilience trainers for the 4th Brigade Combat Team. ?For example, getting stuck at the gate, when a person is late for formation or a family readiness group meeting, may bring on feelings of anger or anxiety and panic. Resilience training gives soldiers and family members the tools that they can pull on when such a situation arises so they do not start screaming at the gate guard or ruin the rest of their day.?

For Sztalkoper, who is also a military spouse, the issue of suicide was not introduced to her by life with the Army.

?As a child of a parent who committed suicide, I only wish that my mother had knowledge of such resiliency skills such as ?Hunt the Good Stuff,? A-T-C (the relationship between an event, our thoughts, and consequences), and put it in perspective,? she said. ?I can only speculate on whether these skills could have been helpful in giving her a new perspective on her life and how to deal with the challenging moments.?

Soldiers need support from their leaders, battle buddies and families, a group McConville calls the ?Golden Triangle.? With that in mind, Fort Campbell and 101st Airborne Division leadership has taken a holistic approach to preventing suicide within the ranks, by including families, Department of the Army civilians and outside organizations such as the USO Fort Campbell in the efforts.

?We didn?t want education to simply stop with the soldiers and families,? said JoJo Huber, health promotion officer for the community health promotion council. ?Through programs offered by Army Community Services within the Directorate of Family and Morale, Welfare and Recreation, we knew we could reach spouses and family members and teach them the warning signs and how to deal with their own stress as well. Reaching out to organizations on post like USO Fort Campbell seemed like a natural next step because we need our community to support our efforts as well.?

USO Fort Campbell came through with a new program offered in October for military couples. The Oxygen for Your Relationship seminar has been featured at military installations across the nation as part of the USO?s Warrior and Family Care mission.

?At USO Fort Campbell, we are continually exploring new ways to support our community. We?re excited to bring the Oxygen for Your Relationship train-the-trainer and seminar to Fort Campbell as part of our mission to lift the spirits of America?s Troops and Families,? said Kari Brown, director of USO Fort Campbell. ?The seminar includes a relationship assessment, the intensive training session, and an ongoing support network. This support network is a particularly important component, as it includes access to a virtual resource center that includes training videos, articles, and live-streaming videos of experts so couples can continue strengthening their relationship after they leave the seminar.?

The USO Fort Campbell will begin offering the seminars to military couples in the Fort Campbell community starting this fall.

One of the more traditional aspects of suicide prevention is the medical community?s involvement, and ensuring prompt access to quality health care is an essential component to the Fort Campbell community?s approach to suicide prevention. Recently, embedded behavioral health teams, a mixture of active duty and civilian personnel, were implemented at the 1st and 4th Brigade Combat Team footprints, and offer services within close proximity of soldiers.

?Placing embedded behavioral health assets in the brigade footprint creates ease of access for junior enlisted soldiers who may not have their own transportation, and it also demystifies the image of a mental health clinic by simply being there,? said Capt. Ashley Chatigny, 4th Brigade Combat Team clinical director for embedded behavioral health. ?The added bonus for commanders is that they now have a relationship with these professionals who can help them deal with concerns about their soldiers or personnel.?

Making behavioral health care convenient is critical, Chatigny said, and that is why the medical community at Fort Campbell and within the 101st Airborne Division has decided to embed health professionals so close to where soldiers spend so much time.

?We are working to fill positions so that all the brigade combat teams can stand up embedded behavioral health teams in the near future,? she said.

Soldiers, Department of the Army civilians and family members deserve healthy, supportive environments and the Fort Campbell and 101st Airborne Division community is committed to raising awareness of the tools and resources available to prevent suicide and increase resiliency.

Fort Campbell and 101st Airborne Division soldiers save lives every day in combat. Standing shoulder-to-shoulder, this community can save lives too.

(AP) ? The infection is as grim as it sounds: "Zombie bees" have a parasite that causes them to fly at night and lurch around erratically until they die.

And experts say the condition has crept into Washington state.

"I joke with my kids that the zombie apocalypse is starting at my house," said Mark Hohn, a novice beekeeper who spotted the infected insects at his suburban Seattle home.

Hohn returned from vacation a few weeks ago to find many of his bees either dead or flying in jerky patterns and then flopping on the floor.

He remembered hearing about zombie bees, so he collected several of the corpses and popped them into a plastic bag. About a week later, the Kent man had evidence his bees were infected: the pupae of parasitic flies.

"Curiosity got the better of me," Hohn said.

The zombie bees were the first to be confirmed in Washington state, The Seattle Times reported (http://is.gd/ji7UNX).

San Francisco State University biologist John Hafernik first discovered zombie bees in California in 2008.

Hafernik now uses a website to recruit citizen scientists like Hohn to track the infection across the country. Observers also have found zombie bees in Oregon and South Dakota.

The infection is another threat to bees that are needed to pollinate crops. Hives have been failing in recent years due to a mysterious ailment called colony collapse disorder, in which all the adult honey bees in a colony suddenly die.

The life cycle of the fly that infects zombie bees is reminiscent of the movie "Alien," the newspaper reported. A small adult female lands on the back of a honeybee and injects eggs into the bee's abdomen. The eggs hatch into maggots.

"They basically eat the insides out of the bee," Hafernik said.

After consuming their host, the maggots pupate, forming a hard outer shell that looks like a fat, brown grain of rice. That's what Hohn found in the plastic bag with the dead bees. Adult flies emerge in three to four weeks.

There's no evidence yet that the parasitic fly is a major player in the bees' decline, but it does seem the pest is targeting new hosts, said Steve Sheppard, chairman of the entomology department at Washington State University.

"It may occur a lot more widely than we think," he said.

That's what Hafernik hopes to find out with his website, zombeewatch.org. The site offers simple instructions for collecting suspect bees, watching for signs of parasites and reporting the results.

Once more people start looking, the number of sightings will probably climb, Hohn said.

"I'm pretty confident I'm not the only one in Washington state who has them," he said.
